When is microneedling not recommended?

Microneedling is an effective treatment, but there are certain circumstances in which it should be avoided and may not be the ideal choice:

  • Active skin infections and skin injuries: If you are suffering from acute viral or bacterial skin infections such as severe acne or herpes, you should wait until your skin has healed before undergoing microneedling treatment. We recommend the same for open wounds. If you are prone to keloid formation, microneedling can increase the risk of scarring or irritation. We will be happy to advise you on site and clarify whether microneedling is suitable for you.
  • Pregnancy, breastfeeding: During this time, the skin is particularly sensitive and the risk of irritation, hyperpigmentation and infections is increased. In addition, many skin care products may be unsuitable for pregnant or breastfeeding women during treatment. We therefore recommend waiting until after the breastfeeding period before undergoing microneedling.
  • Medication and medical treatments: Individuals on blood thinners, certain acne medications (such as Ciscutan) or undergoing chemotherapy/radiation therapy should avoid microneedling. These medications and therapies can slow down wound healing and increase the risk of infection.

For an optimal effect, an average of 4-8 microneedling treatments with an interval of 4-6 weeks between treatments is recommended. If microneedling is not the right choice, at GoAesthetic we offer safe alternatives to best meet your skin needs. Can you do microneedling at home?

Microneedling at home is possible in principle, but carries considerable risks and is not recommended from a medical point of view. The home devices required for this, such as dermarollers or dermapens, can be purchased easily and cheaply, especially online, but generally have shorter needles (0.1 to 0.3 mm) compared to medical devices used by professionals. Some inexpensive devices even have similar-sounding names to well-known medical microneedling pens.

However, despite their easy availability, home devices harbor several risks. In addition to limited effectiveness and inferior quality, hygiene deficiencies and incorrect aftercare are also serious risks.

Limitation of DIY devices with short needles

At-home microneedling devices are often designed to improve the absorption of skin care products by lightly puncturing only the top layer of skin (epidermis). Although they are relatively safe, this surface treatment is not sufficient to trigger deeper skin regeneration and collagen production, as is the case with professional microneedling, due to the short needle length. The effect of such DIY devices is therefore lacking.

Medically inappropriate use

Although some needling devices have longer needles – sometimes even longer – there is a risk of considerable skin damage if they are used incorrectly, which can lead to permanent scars. This is because the needling depth must be adapted to the face and facial region. The increasing risk of infection should also not be neglected.

Poor hygiene conditions

Inadequate hygiene requirements for microneedling at home mean an increasing risk of infection. Improperly disinfected needles or reused attachments pose a huge risk of infection. In professional treatments, every session is carried out with sterile disposable needles. In addition, our medically trained staff always use new sterile surgical gloves. In contrast, unsafe disinfection methods are often used at home. Repeated applications with the home device also cause the needles to become blunt, which can lead to micro-tears and scars.

Dangers of unsuitable products after microneedling

Many people use unsuitable products after microneedling at home that were not developed for damaged skin. Products with silicones, essential oils or fragrances can penetrate deeper into the skin through the open microchannels and cause intolerance or inflammation. In the worst case, this can lead to granulomatous reactions in which the body tries to encapsulate these foreign substances, which can lead to long-term skin problems.

What do you have to consider when microneedling?

Before treatment:

  • No active inflammation in the treated area (e.g.: fever blisters). If you are prone to fever blisters, we recommend medication prophylaxis two days before the microneedling.

  • Some blood-thinning or anticoagulant medications should be temporarily discontinued after consulting your doctor.

  • If you have recently had a wrinkle injection, it should have been at least two weeks ago before the medical microneedling is carried out.

After treatment:

  • No sports on the day of treatment.

  • No scratching or pressing of the treated skin.

  • No massages or cosmetic treatments on the face for 2 days.

  • Be sure to apply sunscreen with SPF 30 outdoors for 5 days to protect the skin from UV rays.

  • No steam bath and sauna visits for 1 week.

The number of microneedling treatments varies depending on the treatment goal. On average, 4-8 microneedling treatments are necessary, with a break of 3-6 weeks between treatments to give the skin sufficient time to regenerate. For specific goals, such as the reduction of scars and stretch marks, we recommend 5-8 treatments, as these indications require longer and more intensive treatment. Refresher treatments once or twice a year can be useful for long-term skin care.

The duration of the effect of microneedling depends on the number of treatments performed and the treatment goal. As a classic anti-ageing treatment, the effect usually lasts for several years once a sufficient number of treatments have been carried out. In the case of scars, especially sunken scars, the effect can even be permanent, as microneedling improves the skin structure in the long term.

Laser skin rejuvenation often involves an ablative procedure that specifically damages the surface of the skin to stimulate the production of type III collagen. This method results in fine, usually invisible scars that contribute to skin tightening. In contrast, microneedling promotes the production of type I collagen, which plays an important role in skin structure and makes up 90% of the collagen in the body. This gentler method improves the appearance of the skin without damaging the surface and thus supports natural regeneration.

Usually not, but this depends on the subjective feeling and the desired treatment: Acne scars or skin rejuvenation? Stretch marks or wrinkles? Depending on the treatment, we need to change the depth of the needles. But: Traditional techniques with dermarollers are often more painful than the Dermapen 4™ – despite the same penetration depth. As the fine needles of the Dermaroller penetrate much more slowly than those of the Dermapen 4™, which is used in our institute in Vienna. And beware: treatment with dermarollers on the face is not recommended, as fine scars can occur if used incorrectly. Nowadays, from a medical point of view, only electric pens should be used for facial microneedling.
